Adaptive designs for selecting drug combinations based on efficacy–toxicity response

چکیده
We propose a new adaptive procedure for dose-finding in clinical trials with combination of two drugs when both efficacy and toxicity responses are available. We model the distribution of this bivariate binary endpoint using the bivariate probit model. The analytic formulae for the Fisher information matrix are obtained, that form the basis for derivation of the locally optimal, minimax, Bayesian, and adaptive designs in the framework of optimal design theory. © 2007 Elsevier B.V. Bayesian dose-finding designs for combination of molecularly targeted agents assuming partial stochastic ordering
Molecularly targeted agent (MTA) combination therapy is in the early stages of development. When using a fixed dose of one agent in combinations of MTAs, toxicity and efficacy do not necessarily increase with an increasing dose of the other agent. Thus, in dose-finding trials for combinations of MTAs, interest may lie in identifying the optimal biological dose combinations (OBDCs), defined as t...
متن کاملBayesian Adaptive Dose-finding Based on Efficacy and Toxicity
A description of the conventional paradigm for early phase clinical evaluation of a new agent is given, followed by a list of this paradigm’s logical and practical flaws. This is provided initially to motivate the use of phase I-II clinical trial designs. The main body of the paper consists of a review of several practical Bayesian phase I-II designs for sequentially adaptive dose-finding based...
متن کاملPrediction of toxicity of aliphatic carboxylic acids using adaptive neuro-fuzzy inference system
Toxicity of 38 aliphatic carboxylic acids was studied using non-linear quantitative structure-toxicityrelationship (QSTR) models. The adaptive neuro-fuzzy inference system (ANFIS) was used to construct thenonlinear QSTR models in all stages of study. Two ANFIS models were developed based upon differentsubsets of descriptors. The first one used log ow K and LUMO E as inputs and had good predicti...

BAYESIAN ADAPTIVE DESIGNS FOR EARLY PHASE CLINICAL TRIALS Publication No. Chunyan Cai, B.S. Supervisory Professor: Ying Yuan, Ph.D. My dissertation focuses mainly on Bayesian adaptive designs for phase I and phase II clinical trials. It includes three specific topics: (1) proposing a novel two-dimensional dose-finding algorithm for biological agents, (2) developing Bayesian adaptive screening d...
متن کاملPractical implementation of an adaptive phase I/II design in chronic myeloid leukaemia: evaluating both efficacy and toxicity using the EffTox design
Despite known limitations of algorithm based designs (e.g. 3+3) they are favoured for use in dose finding studies due to simplicity and familiarity. Bayesian adaptive designs can overcome these limitations as they’re more efficient and substantially more accurate. EffTox is such a design which aims to determine the optimal, tolerable and efficacious dose. We implemented this design in a multice...
